This Health Centre offers a full time Health Care Coordinator (HCC) who, in conjunction with the Aboriginal Diabetes Initiative (ADI) program, organizes and delivers community diabetes programs offering limited diabetic management and nutrition education and follow-up, home visitation, monthly diabetes education sessions, diabetes support group meetings, contracted foot care, and walking/aquacise programs. The HCC program is available to manage home care needs of diabetic patients (i.e. homemaker services, monitoring, wound management etc.). A massage therapist is available weekly to support the needs of diabetic patients. A full time Community Health Nurse is responsible for school prevention programs and chronic disease screening clinics. In addition, ADI prevention and promotion programs are available on demand, with limited financial assistance for non-benefit diabetes-related supplies (on a case-by-case basis). Also, a medical transportation service for in-town and long distance appointments is available. A part time Registered Dietitian assists in diabetes management and nutrition counseling, school education programs for diabetes prevention, Headstart and daycare education for staff and parents regarding healthy nutrition for children. There is also a Healthy Learners School Initiative.
